
Every retired member is required to designate a beneficiary of their choice on their SFERS Plan to ensure a death benefit is paid to the intended beneficiary upon the member’s death.
To designate a beneficiary or update beneficiary information, members can download a
Designation of Beneficiary Form from the SFERS web site; however,
the completed form must be submitted to our office in person. We require a picture ID (e.g. Driver’s License) and the complete names and addresses of the person(s) you wish to designate as your beneficiary. An appointment is not required to update beneficiary information.
